Service Description
Book a JotForm session to create or update a custom form for your business, nonprofit, event, intake process, registration, order form, or payment form. This session includes form setup, basic customization, and guidance on how to use your form.
Cancellation Policy
Late Cancellations / No-Shows A cancellation is considered late when the appointment is cancelled less than 24 hours before the scheduled time. A no-show is when a client misses an appointment without cancelling. Cancellations made less than 24 hours in advance will incur a 25% cancellation fee based on the total service cost. Please be respectful and provide at least 24 hours’ notice for any cancellations. Consultation Policy – “Let’s Chat & See If We’re a Good Fit” To ensure appointments are respected and time is reserved properly, a consultation fee is required when booking the “Let’s Chat & See If We’re a Good Fit” session. This fee will be fully refunded after the appointment is completed. The purpose of this fee is simply to ensure that scheduled appointments are honored and to reduce no-shows. If a client does not attend the scheduled appointment without notice, the consultation fee will not be refunded.
